It has been an honor and privilege to care for you, your families, and our community over the years. I have been fortunate to build many meaningful patient relationships, which makes this message especially difficult to write. After much consideration, I have made the decision to close my practice.
I am deeply grateful for the trust, loyalty, and memories we have shared throughout the years. For your continued care, all medical records will be securely transferred to Northwest Women’s Center. They will have your complete medical history and can be contacted at:

If you choose to establish care with a new provider, please have their office send a records release request by fax directly to Northwest Women’s Center.
For cosmetic records, including pellet therapy, requests may be submitted via email to Houstonreveal5@gmail.com. Please note there is a $40 processing fee, and records will be mailed upon request.
